The referral process is pretty straightforward.  Please download and fill out the referral form and fax it to our office.  We will contact your patient to obtain additional demographic and insurance information, and then set up an integrative sleep consultation or home sleep study appointment as necessary.  To bypass our telephone tree please press #2 to speak with our front desk.
